Donald P. Kent memorial lecture; On adding insight to injury

I wonder if I could invite people who are standing in the doorway to make up their minds. We with the hope that you would come on again. But if you don't would you move on out and perhaps close at least most of the doors. We. Have come. Today to hear. The lecture by one of the persons whom the generalized society has chosen to give one of its distinguished award. For the purposes of those of you. Who may be new to the society I would like to take a brief brief moment. To describe the fact. That the society has an awards committee. That committee is charged with the pleasure as well as the responsibility.
I might distract this for just a moment to say that I myself have acted in a variety of roles in the society over the past. Years and the one role will of course give me the greatest pleasure in the role of acting in the awards selection. But again for those who do not know what the awards committee itself made out. Of past winners of the Queen my are awarded which was the first of the two warrants a society created and then more recently we have now representatives who on that committee who have been themselves winners can't award. So that is presumably a group that has no vested interest in making the selections. I want that merely to be in your consciousness if you have a question about the download. He can't Memorial Lecture is given and hour.
Memory Kant who was a leader in the field he was an enormously energetic man who did a great deal to further the cause of aging. He was an officer of the society and his untimely death was a very great loss to the community of persons interested in this field. The award itself is given to the person who in the eyes of the committee after consultation with a relatively large number of persons whom we do consult. For the following and I call it is and we will award every lecture by a member of the journal logical society who exemplifies the highest standards of professional leadership through teaching service or the interpretation of gerontology to the larger society. In today's winner of that war and I think our choice will speak for himself in a moment. But I want to tell you very briefly about him in case you
don't know. Those of you may not have been present last year when we made a formal official statement on this goal but after Weinberg is presently the director of mental institutes for mental health of the Illinois Department of Mental Health. Why the ten years prior to his newest position he was the clinical director on a high stakes Psychiatric Institute. Now that is merely his most formal of title. I want to tell you without going into detail that if you could think of anything and be helpful it is the Chicago metropolitan area or the state of Illinois and I'll and maybe have the things at the national level which I've been involved with service organization forwarding teaching activities in the field I'm not all for older persons. You will find that Dr Weinberg was either its founder as president on one of his major Mormon or is handled very list itself. Cover Some two pages of

have. And I know how much time I have and I'm reminded of Sunday evening. When the recipient of the chemo award. Dr. Andrus. Gave his lecture. Stated. That he was given 50 minutes. Familiar figure of to a psychiatrist. He said if you remember. He said he has 15 minutes to lecture and you had 50 minutes to listen. And he had hoped to book a few. Both would end in the same time. I. Don't know how much time I have but. I think approximately the same time but I hope to finish ahead of you. I was rather envious. Of Dr.
Andrus. Envious of the fact that he had slides to show. And that no one but no one has thought to ask of me. Do you have any sly. Yes. And I was going to surprise the society. I was going to come with a slight. Depicting a patient. Lying on a couch. With a group of us a psychiatric team. Infusing. Ego. If you'll pardon the expression and extracting good. I had it all prepared until I had to point out. The seat of the good. I didn't know where that was in the brain can depend creosote in the genitals. And to be designated. As they want to present the first lecture. To commemorate that occasion. I am however profoundly saddened. That it has to be a memorial lecture for that very admired respected colleague and friend. My wife who is in the audience can tell you how much I cared and I used about my
presentation. I agonized as it is. But I agonized particularly loved so much as to whether what I had to say would do Miano but whether what I had to say would do I know to. Done camps. And I hope that in a certain way. I will be able to do so. The gentle logical society has within a brief period of time. I think about a decade established to annual awards and lectures in the name of two of its most distinguished and charismatic members who died at the peak of their productive years. Years as academicians. Eloquent articulators of the concepts and concerns of gerontology and most effective leaders who were forwarding its causes in the community at large. There Robert Klee my own award for research in the Donald P. Kanter award
for teaching service over the interpretation of gerontology to the larger society. May I share some of my feelings with you regarding it. I love these type of remarks that speakers make. May I share with you. Who the devil is going to stop me. Since I have to flow. It seemed to me that a dichotomy of two cultures science and humanism. Research and service. Through the establishment of these two awards may thus have been further legitimize. I speak of two cultures. First because each is in fact a system with an internal logic and consistency. Each is built upon a set of assumptions and my prophecies which hangs together and is viable under some conditions.
Second a centrist position is difficult to maintain not only because one may face the danger of either another wanding being considered an outcast of both. But also it is not too easy to withstand the ruling so fervently sought by the seeming polarities with their demand for total commitment. Even when committed to one. Let me say research. The specifics and investigator plans to observe how determined not only by his convictions regarding what variables are pertinent to the phenomena under study but also by the kinds of changes that personally interest him most. The idea that it is more productive to collect data that can be quantified or treated mathematically then data are not lending themselves to such measurements is a value orientation that can lead an
investigator to examine only certain quantifiable kinds of alterations or change. It is a value determination. In contrast an investigator who feels the rigor and logic of mathematics to be inimical to the understanding of the most important personal dimensions or psychological phenomena may select for study variables that are not readily conceptualized in any conventional mathematical frame of reference. It is even possible that neither investigators will pay much critical attention to the other investigators focus of attention. They may simply not pay attention to one another. I do not at this very moment know. Who it was who stated that quote the death of one man is a tragedy. The death of many is a statistic close quote.
What then was I to deal with as an individual tragedy. Statistics about. This abduction of a centrist position is a phenomena that can occur only in societies fundamentally United. At the risk of being considered as one who assume equivocating stances and lacking in convictions and thus appearing to be vulnerable. I nevertheless was ready to take on a centrist post especially you know. Was it given to logical society that provides for us all a form of its expression. Added to the above the impending celebration of the Bicentennial and of issuing of our nation it would. And some of my preoccupation with it. Further invited me to assume what may seem to be a neutral stance. May I refer you. To George Washington's farewell address
upon the completion of his services to our nation as the first president of the United States. It has been described and often referred to as an isolationist position. But which to my mind conveys a conflict between ideals and practicality with subtle in this guy's psychological content. I'm referring to the section in which Washington rules the positive and negative feelings towards another nation can untangle house just as much as informal alliances. Nothing is more essential. He wrote as I quote there are that permanent investment and to parties against particular nations and passionate attachments for others should be excluded and that in place of them just an ABN feelings towards all should be. Cultivated.
The nation which indulges another in habitual hatred O.N. habitual fondness is in some degree US me. It is a slave to it so they must city or to its affection close quote. What I had not expected. That my reading of Washington's message would lead me to a definite conclusions about desirable professional stances to be assumed it had nevertheless further sharpened my personal in philosophical convictions. Namely. And not too surprisingly. To the barricades and off of center. Lovers and haters of values I was prone to penalties as are lovers and haters of nations. Principles have to be paid for. Nothing comes free. This being so it was incumbent upon me to decide which values were important enough to me that I would
check if I used other goods for them. It turned out from my introspection that though I love science my strongest feelings by far were for humanism as juxtaposed to science. If I decide a value of purpose in life. If I did not want to merely exist on an emotional dead center. If I wish to continue to subscribe to Justice Holmes dictum quote life is action and passion. I think it is required of a man that he should share the action and passion of his time at peril of being judged not to have lived at all. Close quote. If all of these. If all these Indeed I must be prepared to pay a considerable price for humanism perhaps a career for life itself. Love that price for that value. Then another kind of price.
A lessening sense of vital engagement becoming a passive inhabitant of the land so to speak rather than an involved citizen. Typically my thought processes led me further to further speculation than questions. Was Washington stance that of an old man and that of Holmes young. Was Washington disengaging not only from the presidency but from feelings and ongoing like activities and concerns as well as many of the elderly I want to do. And was Justice Holmes who at that time chronologically older than Washington with his call for passion and action. Was he young. Neither one of there was available to me to explore his unconscious. As to the psychological dynamics. The evocative restoration of
memory of Things Past. So much to examine and develop to the full the individual's cognitive and adaptive capacities through insight. For psychological insight could be operationally defined. As they're temporarily ecstatic moment. Exquisite moment in which patient and therapist believe that they share the same perception as to the cause and therefore possible and million aeration of the difficulties encountered. His is a system spiraling constantly to levels of more organized complexity. Meaning of Cosmo differentiated emotions cognitions and patterns of transaction. It is this differentiation to which he is encouraged and himself aspires. That fashions men into a unique subsystem. With a super power system. All components of the Super System attempt to mold a human being into a conformity. Can Durant and in concert with its ordained order
discourage individual uniqueness. And thus more often than not I win Julius to the free spirit of man if not his if not his physical self. The injuries that life deals to the individual are practically inevitable and in explicable to him. His own to his interpretation of events may or may not be faulty. He's need for understanding and insight into his behavior and meaning of existence compelling. To suffer injury in a state of innocence is doubly difficult and I'm living. Our efforts my efforts. I would attempt at providing that understanding if not in video ration of pain. The taking of photographs. Makes me think. As you will perceive throughout my
talk. Of my plea to be concerned with the individual. And what I like to call with the legend that existence in the human being not the image which is the concretize thing that we present to other human beings. I don't know about you but I've had hundreds of photographs taken of me and I have yet to see the picture that does me justice. And I hope and I don't think that you're laughing at me but with me. And I look at myself in the mirror every day when I shave and I don't believe what I see. Because I know what I look like. a reasonably open mind can take us some way towards conclusions which may have a certain validity. What is essential is to retain a multi sided approach to each fragment of material available so that there is no undue preoccupation with one facet to the obscuring of the others and also that each chain of association and inference should be built up so far as possible without piling hypothesis and hypothesis so that bio hypotheses take on the character of established fact rather than conjecture will breaks in an unsubstantial construction. Well if this is done at any time it should be made explicit. What I'm doing of course is to defend speculated formulation provided it does not pretend to any exclusive revelation and remain attached to some documentary evidence. To understand
the uniqueness of each person's reality. The Young and the old is the very essence of psychological skill. Skill which must deal not only with the individual variance but also with the shifting and altering quality of the subjective state as the human being grows develops and ages. Each period in the life span of men forced on him a different reality based on the altered physiology and the extent of the richness of the human experience. Old age as you know can become an expression of the summation of human experience. It can be rich. It can be varied it can be colorful and in turn and witching. Or conversely it may be impoverished or empty and only served to emphasize the futility of life and its meaning to many of the old. However no matter what one's experiences may be there is a need for an
elaboration upon one's experiences which tend to distort the objective truth but which add to the uniqueness of the reality that the individual wishes to convey. It is by far more real for weeds for he delineates more clearly to the observer the actual personality of the observed. It is more real of a personality structure of the communicating person than the actual facts may warrant a state. What emerges is of a mental size version of events. A dramatization of the facts. The good can become magnificent the bad. Unexceptable tragic. Again a poetic interpretation of one's experiences which in turn need interpretation by the listener. Also we were engaged in the field of mental health. And health in totality are quite aware of the permutation of time.
We work with emotions that have their origins in the person's dim forgotten where we press past. But which make themselves financially felt in the present. No sooner does the patient a human being experience an emotion. A feeling then it is immediately related to his past he's dreams no of no time dimension past present and future permeate all three and up to eight as that individual organism has no cognizance of that which we know as being temporal. What is needed most careful in working with emotionally you know elderly is an understanding of the manifest behavior in the light of their lifelong experiences and affected as it may be by a crude deficit's both extrinsic and intrinsic. good coverage over the seemingly bizarre understandable to point out it were inherent in chaos and thus make life bearable for the observed and help the observer. Behavior is the aggregate of observable responses of the organism in its entire relationship in its predictable form. It is the usual modality by which an individual handles his life situations that may arise. It is usually automatic reflex it and more often than not defines the character of the person. It is what one expects of one's contact with another and upon which one projects a continuum of transactions. While my no deviations from the established patterns are allowed for under certain circumstances. The usual set of expectations are those of stability if not rigidity. Unfortunately
the predictability of behavior presumes a static quality to the source the human being who of course is in a constant state of change development. And hopefully growth. Added to this dilemma is to suck. The subtlety of an ever increasing spectrum of that of that which is called No no. Do both to the greater tolerance of society by society of more and more deviant behavior and the greater understanding that the helping professions bring to the study and alleviation of the human condition. In view of the above I have chosen to again speak to the psychodynamics of what I like to call aged less weather than aging aging as has been repeatedly stated is an ongoing process therefore difficult of assessment aged. This however is an assumed stance on the part of the organism that may or may not be due to organ dysfunction
but may be be behavioristic leap characteristic of a unified complex of those assigned descried and much too often acquired. It may be both chemical and pathology. It may be independent of any overt manifestation of organic disease and thus present itself as a mode of coping. No adaptive behavior most economical to the character structure of the individual. Behavioristic least speaking one may manifest hipness quite early in one's life though usually it is. Characteristic of the later period of the life cycle. Thus it is the manifest behavior of an individual that should interest us and the latent meaning of its content. That should intrigue every one of us. In any disease. We must ask ourselves three questions. These are what how and why.
Do what is the definition of the disease and its position in a nose a logical classification. The howl refers to the multiple causes ending in the disease. And why is TV your logical teleological or the meaning of the disease as an adaptation. This last question is usually shown by scientists but according to foundry. The behavioral modality is subsumed under our diagnostic category are
not describable as unitary. It is not the diagnoses that have a reality of reference but rather the behavioral variation the diagnoses described in any type of class or behavior one will see extremes and the law and the laws that govern and describe those extremes also govern and describe the non pathological functioning. Thus the idea of continuity between normal and abnormal can be establishments as having me and easily found. We can further assume that normal behavior is unique to a diagnostic category. Therefore individual symptoms are not reliable indices for psychopathological grouping when precise and definite. If your logical factors are isolated and the pathogenesis is well understood the disease entity itself may certainly be used to define a target population.
However most studies on the psychopathy logical condition of aging do not define a theological or path of pathogenic factors soley and liking such forces leads some of us to what may be described as empirical contusions and behavioral patterns. I know that many of my colleagues disagree with me. However since I'm really talking of a personal journey so to speak. That has brought me to. My functioning as I do. I must of course describe to you what it is that I do. And hence all of these remarks which are lengthy. Perception psychopathology relationship in old age is a case in point. While some regard perception as an independent variable and direct our attention to the effect of disordered
perception and personality development and consequent behavior others if humans psychopathological conditions as independent variables in regard to perceptual behavior as outcomes. Of effects. I for one am minute here and to the latter notion having long since postulated. To some of you may be familiar. My notions about the exclusion of stimuli in later life as an outcome or effect of a psychopathological. If not psychodynamic condition. Perception of course refers to a perceptual act that transforms a physical stimulus into psychological information. Complex processes are involved in this transformation. Reception of the stimulus registration the processing of the registered information in the checking of the information against continued input. Eventually the human
organism o organization is meant to interpret all of the above in the light of its life experiences. And effect action. Thus sensory cognitive conceptual affectation motor processes are all linked with each other in any given perceptual act. Most of us have long since recognized the crucial significance of perception its central role in the development of those modulatory and controlling structures designated as the ego or as I like to call it the problem solving self. For the perceptual act reflex to psychological point of contact between a person and his internal and external milieu. Its principal function is to convey information from his environment for integration with other psychological functioning such as memory judgment and anticipation. Obviously too it also receives and carries information about the
nature in consequences of the perceivers actions. Perception then is a central ingredient in effective adaptation in the fitting in process between the individual and his environment. As the individual develops and moves towards Ekta mastery. He can no longer depend on the instrumentality of others as he does in infancy childhood in adolescence but must employ Phi in coordinate the executive potential of his own body parts which cease to be independent information enclosure preceptors and take up their collaborative productive function. Many have carefully pointed out that perception is not a passive process. A physical stimulus is organized and transformed at the point of perception at the point of entry. And I like to believe that it never goes beyond that point. Beyond the point of the sensory receptor in order to be. Effective. And in order to provide the individual with some power stimuli. The infant too is faced by the same problem of too many stimuli with little ego development to help it cope with them. However the very young can and do take refuge in sleep or withdraw to allow for a gradual exposure to the clamor and the slow major developmental integration of the stimuli and the evolvement of acceptable
responses to them. Then too they have the help of supportive figures who are ever ready to supply ego judgment and strength to this troubling new organism. Both of these elements are not available. No are they acceptable to the ageing individual and hence its exclusion. Exclusion of seniors though it may be argued by some that the mechanism of exclusion of stimuli is identical with the familiar mechanism of denial. It is my belief that this defense is rather difficult. Different denial to me employees that a stimulus has been received affected too invested in and then cathexis with not so with the exclusion of stimuli. It may be blocked at the point of entry with lower threshold only for those stimuli relevant to one's narcissism. And by your letter at this point I mean the survival value. The problem as one may see it is how to experimentally assess the rate and
extent of the exclusion so as to utilize this mechanism as a psycho biological measure of the aging process itself. Based on the above assumptions and they are on and on the assumption that our society is neglectful if not hostile to the needs of the aged one may allow oneself such dynamic formulations on some aspect of their behavior. The sensory organs again do and develop. Each individual with personal spatial boundaries boundaries within which messages may be perceived and which may differ in dimensions in scope for each organ. Tactile language would therefore be the closest and I less invited eagerly your info. Maybe the most encroaching unthreatening of all stimuli received thermal all factory all and visual stimuli in that order provide ever increasing spatial territoriality for meaningful
messages of increasing complexity to reach the human being. And as I perceive them to be what indeed are the characteristics and traits of the older person which make them especially vulnerable to fallacious claims and promises and actual thoughts one might be tempted to ascribe the ready susceptibility to faulty judgement so characteristic of those who manifest symptoms of a chronic brain syndrome and perhaps most of it is. However that is not the totality of it. The eternal search for the fountain of youth its robotic values empower a literal mythological symbolic and scientific has not been the prerogative of the old but has more often than not been in the hands of the young who wish to maintain their status quo either to magically or scientifically stop the time clock or reverse it. Men have literally lost their lives and then they go likely their souls to find to attain eternal youth while all existing knowledge is valiantly and vainly

That is to be more anxious than to be knowledgeable. Biological deficits usually accompanied by over compensatory mechanisms do result in ever reserve judgement which may certainly make the elderly vulnerable to any promises of easy reversal of the ills or the easy restoration of their powers. Be it physical or psychological fantasy
magic wish fulfillment. All too familiar coping mechanisms replace whatever good judgment one may have possessed. So should one's health and mastery be threatened. And threatened they are. In a culture and so the social structure which are inimical to one's interest and hostile to one's legitimate complaints. I have a few moments. Or maybe I'll skip because I do want to end. Let me just say that following the self-deluding propensity of men of all ages is prodigious and well documented every age phase in the life cycle has its own particular dreams which feed its hopes and make for its particular vulnerabilities.
